What is a FUE Hair Transplant?
FUE Hair Transplant is hair grafts transplantation harvested between two ears or other body parts that are genetically resistant to shedding. This method was invented by Dr. Rassman back in 2002, it got very popular together with micro motor technique replacing from the sector the remaining FUT method. In FUE hair roots are harvested one by one, there are no cuts and sutures. People quickly switched to the FUE once it was introduced to the sector. Today FUE completely replaced FUT method.

How is the FUE Hair Transplant Done?
A patient must be positioned face-down or serjeant. Once the grafts are taken from the sides the patient is placed in a lateral decubitus position. Extracting the grafts from a lay-down position is faster but it causes intense bleeding. Thus sitting position results in less bleeding.
First of all, recipient area should be marked with a regular pen. Planning helps to divide the area equally. The divided sections are inflated with saline which is also called an isotonic solution. Skin swelling simplifies hair extraction and prevents root damage. Once the local anesthesia is applied and the area is numbed, extraction of the hair grafts can begin.
2,5x – 4x loupe ratio punches are used during graft extraction. If the loupes are not used, the grafts can be damaged significantly. Thin punches cut the roots of the hair without the need to suture. Extracted hair follicles are placed in pine containers called Petri dishes and kept in the cooling containers until the implantation. The hair grafts should be preserved at the temperature of 0,4 degrees Celsius.
FUE Donor Area
It is necessary to have the hair shaved for the FUE hair transplant. The hair length on the donor area should be 1mm at the regular FUE operation; it is called Zero shave. Normally, around 5000-7000 grafts are extracted from the shaved zone depending on the need and capacity of the donor area.
Grafts must be harvested from safe areas. Safe area has strong roots and is rich with anagen. Grafts can also be collected from above ear and occiput. This hair is thinner and better suits frontal hairline. Roots on the under vertex area have a high loss possibility, which is why they are not used. There is also a risk of hair loss with the parietal ear hair in the future. A very minimum grafts must be collected from these areas. It is very important for young people.
The pen line serves as smooth transition, there must not be dense transplantation on line. To obtain more natural hair the frontal hairline grafts must be single and thin.
FUE punch diameter is a key factor in grafts harvesting. Thick punch results in limited number of grafts extracted and transection rate is considerably high. It also leaves bigger skin scars. We prefer 0.7, 0.8, and 0.9 mm diameter punches.

1- FUE Punch Size and Depth Control
Punch entry depth is very important in FUE hair transplant. It does not matter if it is manual or micro motor FUE. When the punch is too long hair roots can be damaged during extraction, which is called dissection. It causes unnecessary graft loss. Patients have different graft sizes; the ideal punch length can be determined and fixed after a few tries. In opposite, if the punch is short, it is difficult to pull out the hair, and this causes destruction too.
2- Fue Punch Entry Angle
It is easy to find the right angle after a first try. A wrong entry angle causes transection – graft destruction. There is a difference between the hair growing angle inside and outside of the skin on the nape area. A well-qualified FUE hair transplant doctor can easily identify proper angles and do hair transplant surgery accordingly. Transection scale (destruction volume) is a percentage relation of defected hair roots to healthy grafts. A 2-10% transaction scale is acceptable according to the international standards. FUE Hair Transplant Tools
FUE has two hair extraction methods, that include various diameter tools called the punches. These punches vary as sharp, round, fish- mouth, serrated, and Dr. Umar’ the U shaped. There are many producers of the punches in the market, their diameter ranges from 0,6 to 1,5 mm. In most cases, 0,7, 0,75, 0,8 and 0,9 mm diameter is suitable for graft extraction. Punch length must be selected according to hair size, this is 3 mm, 3,5 mm, or 4 mm.
a) Manual FUE
Hair roots are extracted manually with a thin and sharp punch. There are many different manual punch types in the FUE transplant. Manual FUE extraction is much slower than micro motored. Round and fish mouth punches are used in manual FUE.
b) FUE Micro Motor
Micro motor is located inside of the punch. This tool with adjustable speed works on electricity. Circling the punch extracts hair grafts faster than the manual. However, if the speed is too high, grafts can be heat and damaged. There is a false belief that the micro-motored FUE punch destructs grafts. If used correctly in the FUE hair transplant results are great.
c) Robotic Hair Transplant
The USA has been developing a modern computerized hair transplant device. It is manufactured by Artas company. It is not a complete hair transplant machine. The device marks the grafts and only cuts them via rotation. Than these grafts are gathered manually like in the classic approach. Automated hair transplant increases a damage risk of the grafts significantly. Moreover, the diameter of their punch is generally 1,2 mm, which also reduces the number of healthy grafts. It also leaves notable scars on the skin.
After FUE Hair Transplant (Donor Area)

What FUE Hair Transplant Advantages and Disadvantages?
- Fue transplant more than 2500 grafts is not healthy to transplant in a single session. Operation can be divided into 2-3 consecutive days when grafts number is big.
- One session lasts around 5-6 hours.
- Women completely switched from the FUT to the FUE. FUE offers no shave hair transplant.
- Fue is the best eyebrow transplant method.
- FUE beard transplant became irreplaceable. This is because you can select strong grafts in anagen phase (hair in the development) to transplant.
